このログからわかるのは

VGA16 サーバーで使うために、たぶん /etc/X11/XF86Config ファイルの
Section "Screen", Subsection "Display" に Modes "640x480" が指定されて
いるけれども、同じファイルの Section "Monitor" に HorizSync と
して 31.54 を超える下限が指定されており、かつ Modeline "640x480" ... と
して指定されているモードラインが水平走査同期周波数として 31.54 kHz を
必要としているため、この両者の矛盾した指定によって "640x480" という名称の
モードが削除されてしまっており、 XF86_VGA16 サーバーにとって使用可能な
モードが存在しない設定になってしまっている、ということですね。
XF86Config の設定については xjman project <http://www.dsl.gr.jp/~go/xjman/>
にある日本語 man ページを参考にして下さい。3.3.3 の man ページが暫定公開
されているはずです。もっとも、基本的な部分は岡本さんによる日本語訳とほぼ
同じで、主に Xinput 関連の追加が更新部分でしたけれども。
また特に Modeline の設定についての説明として xjdoc のアーカイブに
modelines.txt というファイルもあります。まだここで公開されている文書は
この ML をきっかけとしておぎそさんに頂いた情報を追加した版には更新されて
いなかったと思いますが、とりあえずの参考にはなるでしょう。
まずは、設定をちゃんと動作するはずのものにしてからですね。
実際にそのチップが使えるかどうかを確認できるのは。
